BOARD OF HEALTH
Meeting Minutes
February 25, 2008
Memorial Town Hall

MEMBERS PRESENT:  Martha Waite, Mark Johnson

MEMBERS ABSENT: Mary Ricker

OTHERS PRESENT: Andrea Crete, Agent, Randy Mizereck, Consultant, Debra Sciascia, Recording Secretary

The Board of Health Meeting was called to order at 5:05 PM.

SEPTIC PLAN REVIEWS:

Chapin Road, Lot 2: Chris Knuth (Geo Services) for John Meyer. Renewal/revision of new septic system.
Conditions:
1. Soil evaluator certificate and signature for soil testing done 9/10/07.
2. Property line must be surveyed and marked.
3. A soil certificate for the use of C-33 sand.
Recommendation(s):
1. The Board recommended that the owner install the well.

M. Waite motioned to approve the plans with the above conditions and recommendation(s); M. Johnson seconded. All in favor. Andrea will send a copy of the approval letter to John Meyer at 45 Chapin Road.

MEETING MINUTES:
• November 26, 2007: M. Waite motioned to approve the minutes. M. Johnson seconded. All in favor, approved.

Chapin Road, Lot 14: The Holden Board of Health is granting the request of a 1 year extension for Lot 14 Chapin Road with the stipulation that the conditions and recommendations from the approval letter dated March 29, 2005 remain in effect and that the septic design plans must comply with revised Title 5 regs including but not limited to:
• magnetic tape needed on all system components
• inspection ports are required
• risers over d-box and septic tank if finished grade greater than 9"
M. Waite motioned to grant the extension. M. Johnson seconded. All in favor.

OTHER BUSINESS:

• 2451 Main Street: For discussion purposes. There will be an addition to the building. For septic plan review and approval of the relocation of the current septic tank and pump chamber, the applicant will need to submit a Disposal System Construction Permit Application along with the appropriate fee and 3 sets of proper septic design plans. The plans should specify whether the septic tank and pump chamber are H-20 loading (both are required to be H-20 loading per 310 CMR 15.226(3) and 15.231 (5) respectively), show the existing/proposed elevations, contours, and elevations of the system components being relocated. If, in the event the septic tank and pump chamber are not constructed of one solid piece, a new septic tank and pump chamber will need to be installed. Set bakcs and easements must also be researched.

Animal Regulations: A. Crete submitted a draft for Board discussion regarding animal/barn regulations. The Board recommended re-working the draft.
Body Works: The Board recommended checking with Town Counsel regarding liability. One letter has gone out regarding performing polarity in town, and this person would be subject to future rules and regulations adopted by the Board.
Sterling Road Septic Review: R. Mizereck and A. Crete updated the Board regarding the status of these systems installed and repaired.
MAHB: Information was distributed to the Board regarding a recommended upcoming class.

Next Meeting: March 31, 2008 at 5:00 PM.
